Forum Health at Home, an affiliate of Forum Health System and a non profit agency, provides personal care, homemaker, licensed practical nurses, and registered nursing services up to 24 hours a day, 7 days a week. In addition, Forum Health at Home provides home delivered meals, congregate dining site meals, transportation to medical appointments, shopping, and Lifeline Emergency Response systems.

Personal care, homemaker, licensed practical nursing, and registered nursing is provided in Mahoning, Trumbull, and parts of Columbiana counties. Home delivered meals, congregate dining site meals, and transportation services are provided in Mahoning County.

Lifeline Emergency Response Systems are provided in Columbiana, Mahoning and Trumbull counties and Western Pennsylvania.

Services provided by Forum Health at Home are funded by PASSPORT, Ohio Home Care (Medicaid), Federal Title III and State Block Grants through District XI Area Agency on Agency, and by personal payment from clients.
